Los Angeles County Los Angeles County proposes to utilize equipped vans staffed with mental health professionals to provide transportation to consumers on involuntary holds to the psychiatric hospital. Wellness in the Streets (WITS) is a peer-run and peer-led project based on the concept that lack of housing should not be a barrier to improving mental health. Wellness in the Streets WITS will test new ways of peer-to-peer service delivery and engagement with San Francisco 10/25/2018 $1,750,000 (WITS) unhoused San Francisco residents by meeting them where they are and supporting participants through the stages of change until they are able to engage in services. In 2016, the City of Berkeley received approval of $180,000 for an Innovation project to provide Trauma Informed Care (TIC) for youth throughout Berkeley City of Trauma Informed Care for 12/17/2018 $266,134 Unified School District. This collaborative project was designed to provide TIC Berkeley Educators training to educators and interested parents and bring TIC practices into the public health, mental health and law enforcement sectors. The Fuerte program is proposed as an adaptation of a promising school-based prevention approach to promote support around acculturation and behavioral health access for immigrant youth, ages 12 to 18. San Bernardino County proposes to utilize engagement and mobile treatment teams to outreach and engage the unsheltered homeless population in their Innovative Remote Onsite San County. Additionally, the County states they would like to test a multi-agency (InnROADS) case management approach to determine its effectiveness in engagement with this population. In 2017, Mono County received approval of $259,046 for an Innovation project to develop a regional collaborative with the neighboring Counties of Inyo and Alpine as well as various community based organizations. The collaborative focused on training County staff and partners on the Strengths Model with the goal of Eastern Sierra Strengths- developing skills for staff in order to provide improved services to clients, prevent Mono Based Learning 3/28/2019 $84,935 staff burn out and integrate the best practice in the three counties. • Mono County Collaborative - Extension is proposing an extension of 4 months for more in-person, 1:1 time with facilitators and is requesting additional funds in the amount of $84,935 (an increase of 32%) for the additional training time and for a more qualitative evaluation in order to complete the project. On July 28, 2016, San Mateo County received approval of $2,200,000 for a 3 year Innovation project to establish The Pride Center in order to provide a coordinated LGBTQ Behavioral Health approach across clinical services, psychoeducational and community/social Coordinated Services events and resources for the LGBTQ+ community. • The collaboration experienced San Mateo 3/28/2019 $1,550,000 (The Pride Center) - contracting and implementation delays in addition to serving twice as many Extension clients in the first year than originally planned. These challenges required an adjustment of the original timeline and the addition of new assessment tools to aid the evaluation. Butte County is proposing to establish a technical assistance center and learning collaborative to serve young children (ages 0-5), their parents and caregivers. The Butte Center Care Project 4/25/2019 $1,671,031 collaborative will facilitate: expansion of access to specialized, trauma-informed, multi-generational, and cross-sector treatment modalities offering community level education and system collaboration. Alameda County is proposing the Mental Health Technology 2.0 Innovation Project which plans to establish a competitive bid process for community based organizations and software developers to work in tandem to develop mental health mobile applications to support four specific populations; caregivers of Mental Health Technology Alameda 4/25/2019 $2,040,120 family members who suffer from a serious mental illness, youth and transition 2.0 aged youth who are victims of trauma, attempted suicide survivors and immigrants, asylees and refugees who are particularly isolated in the County and who may have a set of barriers (i.e. stigma, knowledge, language) that prevent them from seeking services Originally approved on January 28, 2016, Shasta’s CARE Center provides comprehensive and integrated “whole person” after-hours services, including Shasta CARE Center (EXTENSION) 5/22/2019 $289,400.84 family and client psycho-education, peer support, pre-crisis and emergent crisis clinical services, case management, respite care, targeted youth supports, transportation, and linkage to supported housing and other community resources. Orange County will work in collaboration with state and local agencies, public and private health plans, partnering agencies as well as their community to develop an integrated public/private behavioral health system to serve County residents, regardless of an individual’s insurance coverage. Orange County states this project will involve intensive planning and will require the involvement of subject Behavioral Health System matter experts statewide and is intended to explore the feasibility of a system level Orange 5/23/2019 $18,000,000 Transformation change of this magnitude. It is important to note that Orange County states they will return to the Commission to seek funding for additional separate project proposals if the feasibility of this project proves to be successful in merging public and private funding to serve the residents of the County. The overarching goal of this project will ultimately include several interlinked components with the hopes of blending funding from private, public, and philanthropic sectors. Ventura County plans to increase resilience and community building activities, which are known to benefit youth with adverse childhood experiences (ACEs), by Conocimiento: Addressing using a community driven practice called Conocimiento, which combines Ventura ACEs through Core 5/23/2019 $1,047,100 resiliency building and community involvement in two local community centers. Competencies These activities will include community meals, skill building activities, and will be rotated between the two community centers with transportation being provided to facilitate participation by youth. Los Angeles County proposes to test a pilot in the Hollywood region of the county that has the potential to dramatically transform the landscape of the current mental health system. The County is requesting Innovation funds to temporarily replace the entire existing public mental health funding system to test a new payment and documentation system to see whether it will improve the Los Angeles The TRIESTE Project 5/23/2019 $116,750,000.00 effectiveness and satisfaction of mental health services resulting in improved outcomes without increasing cost. If successful, the County plans to expand the model across county services and is hopeful that they can demonstrate to the state of California and the federal government that the Medicaid financing and accountability system should be changed. San Francisco requested additional funding up the amount of $182,850.00 in Transgender Pilot Program Innovation funding authority to increase access to services for underserved groups San Francisco 6/11/2019 $182,850.00 (EXTENSION) by creating social support networks to engage Trans Women of Color into culturally responsive mental health services. Fresno County requests authorization to use up to $750,000 over 5 years ($150,000 per year) to support the Innovation-related Community Program Planning Process (INN related CPPP). The County is proposing to contract with a marketing/communications firm to conduct focus groups on specific populations Community Program (African American adults and youth, Spanish speaking adults, Hmong speaking Fresno 6/24/2019 $750,000 Planning Process adults, older adults, LGBTQ TAY and adults, and caregivers) with whom the County has had difficulty engaging. The County intends to conduct ongoing evaluation of the INN-related CPPP in order to draw from and apply lessons to improve both the INN planning process and the program planning process in the other MHSA components. Fresno County is requesting approval of $950,000 to participate with other Full-Service Partnership Fresno 6/24/2019 $950,000 counties in an evaluation of full-service partnership (FSP) programs in order to Multi-County Evaluation determine if there are any best practice models for successful FSPs. Fresno County is seeking to use up to $950,000 of Innovation funding over 3 years to participate in a Multi-County Innovation Project - Psychiatric Advance Directive (PAD) - Supportive Decision Making.
