Gatekeepers for this project includes childcare providers, cosmetology providers, as well as employees working for medium to large organizations. Stanislaus County requested up to $1,757,146 of Innovation spending authority to join the Full-Service Partnership (FSP) Multi-County Collaborative for existing County specific FSP programs, originally FSP Multi-County Stanislaus 6/24/2021 $ 1,757,146.00 approved by the Commission starting with Fresno County on June 25, Collaborative 2019. Full-Service Partnerships are designed to support individuals requiring services with the most severe mental health needs and co- occurring disorders by incorporating a “whatever it takes” approach. Stanislaus County requested up to $1,564,633 of Innovation spending authority to join the Learning Health Care Network (LHCN) for existing Early Psychosis (EP) programs, a multi-county collaborative approved by the Commission on December 17, 2018. Humboldt County requested up to $1,617,598 of Innovation spending Resident Engagement and authority to test whether assigning case managers and peer coaches to Humboldt 6/24/2021 $ 1,617,598.00 Support Team (REST) consumers, who recently became housed, will help them maintain their housing. Imperial County requested up to $3,455,605 of Innovation spending Holistic Outreach Prevention Imperial 6/24/2021 $ 3,455,605.00 authority to provide holistic treatment options following a psychiatric and Engagement (HOPE) emergency for youth and young adults, ages 13-25. Butte County requested up to $1,252,631.000 in Innovation spending authority to promote interagency collaboration by partnering with the Butte-Glenn Medical Society, Butte County Office of Education and local Butte Physician Committed 6/24/2021 $ 1,252,631.00 primary care physicians in order to expand their ability to identify and intervene with adolescents who have risk factors of experiencing mental health and substance use issues. Santa Clara County requested up to $990,000 in MHSA Innovation funding to create a voluntary supportive network for independent living Independent Living Santa Clara 6/22/2021 $ 990,000.00 facilities (aka room and boards) to include resources for homeowners Empowerment Project (ILEP) (training and education) along with support for client/tenants through Peers with lived experience Colusa County requested up to $498,812 in Innovation spending authority to improve outreach to unserved community members with Social Determinants of Rural Colusa 6/22/2021 $ 498,812.00 mental health needs and to better understand whether and how a social Mental Health determinants of health framework can improve county approaches to prevention, early intervention, and access.
