This project proposes to examine and improve life factors and conditions for outreach to unserved community members with mental health needs, living in rural communities. The goal is to better understand and serve the non-medical influences that impact mental health outcomes in places where people live, learn, and work as well as how social, economic, and environmental factors effect and impact social determinates of mental health. Practical Actions Towards The goal is whether and how a social determinants of Health (PATH) Formerly Social health framework can improve county approaches to Determinants of Rural Mental prevention, early intervention, and access. Colusa Health - EXTENSION 11/18/2022 $983,184.00 This program will establish adult crisis stabilization and intensive mental health support services on a hospital campus (Mercy San Juan Medical Center) located in the underserved and high need northeastern section of Sacramento County. The primary purpose of this emergency care integration innovation project is to demonstrate improved behavioral health outcomes through a public/private collaboration that removes existing barriers to care, increases access to, and the quality and scope of, crisis stabilization and supportive mental health services that are integrated and coordinated. Project services, sited in the Behavioral Health Crisis northern region of Sacramento County, will increase access to Sacramento Services Collaborative 1/4/2023 $1,000,000.00 crisis services for underserved area residents. Bring behavioral health and physical health services to farmworkers and their families, including children, through an outreach team and a mobile health bus that will be equipped to provide clinical, medical, and prenatal care. These services will also be provided in Spanish with the goal of supporting the Mobile Behavioral Health farmworker community to engage in behavioral health services, San Mateo for Farmworkers 2/23/2023 $1,815,000.00 supports, and resources. As a result of this project, MCBH will increase the availability of culturally and linguistically appropriate housing for the adult SMI population by repurposing residential and commercial properties to support residential care Residential Care Facility clients.
